| What Signature Form should I use for my Form 990 / 990-EZ Return? |
| The IRS requires that we attach a properly filled out, signed and dated signature form to your return before we transmit the return to the IRS. Effective with Tax Year 2021, the IRS changed the signature form we are required to attach to the return. Based on the tax year of your return, please fax/email us a properly filled out, signed and dated signature form as follow:
- For Tax Year 2020 (and earlier): Form 8453-EO
- For Tax Year 2021 (and later): Form 8453-TE
IMPORTANT: We cannot accept any other types of signature forms. The form you send must be one of the two forms listed above based on the tax year of your return.
Please note that when you mark your return as complete, our system will create a properly filled out PDF file of your Form 8453 Signature form (as describe above). All you need to do is print it, sign/date it and fax/email it back to us.
You can email a scanned copy (or picture) of the signature form to us at [email protected] or you can fax it to us at 866-699-3916.
Please include your EIN in the subject line or body of the email and try to limit the file size to 1MB. Please do not send via a secure email. Our security policies prohibit us from retrieving files or emails from external servers.
Upon receipt your signature form will be reviewed by our staff, during business hours (M - F, 9:00 a.m. - 5:00 p.m., eastern time), to make sure it has been properly completed, signed and dated by the appropriate person(s). If everything is OK, we will attach the signature form to your return. You will receive an automated email from the system when your signature form has been attached to your return. If there is a problem with the signature form, we will send you an email telling you what the problem is and how to correct it.
You will find information here on how to view, download and print the Form 8453 signature form that the system created for your return once you marked your return as complete.
