How to send the 8453 Signature Form by email or fax.

There are four methods you can use to send the signature form: You can:
  • scan the form and email it to us at [email protected]
  • fax it to our toll-free fax number: 866-699-3916. If you cannot get through to our primary fax number, our second fax number is 888-561-2771.
  • take a photo of the form with your phone and email to:[email protected]. The image must be clear and complete
  • Use a fax service. Take the paper copy of the form to an office supply store such as Fed-Ex, UPS Store, Office Depot, etc and have them fax it to 866-699-3916.


Please include your EIN in the subject line or body of the email and try to limit the file size to 1MB. Please do not send via a secure email. Our security policies prohibit us from retrieving files or emails from external servers.

Upon receipt your signature form will be reviewed by our staff, during business hours (M - F, 9:00 a.m. - 5:00 p.m., eastern time), to make sure it has been properly completed, signed and dated by the appropriate person(s). If everything is OK, we will attach the signature form to your return. You will receive an automated email from the system when your signature form has been attached to your return. If there is a problem with the signature form, we will send you an email telling you what the problem is and how to correct it.

You will find information here on how to view, download and print the Form 8453 signature form that the system created for your return once you marked your return as complete.
