The web page provides a list of valid state forms that you may file along
with your Form 990 or Form 990-EZ with the IRS, or independent of filing the
Form 990 or Form 990-EZ with the IRS.
The web page displays only those state forms available for you to file.
It may show no forms or a list of one or more forms. There are several reasons
a form may not be shown here. Below are the most common reasons.
- You have already filed the specific state form for the tax year (applies to all state forms).
- You are not filing a Form 990 or Form 990-EZ and the state form MUST
be filed at the same time as a Form 990 or Form 990-EZ
- The state form can be filed only once, NOT YEARLY, with the
state and you have used efile.form990.org to previously file the state form
- Some state forms cannot be filed along with the Form 990-EZ or Form 990.
- You are filing the state form along with the Form 990 or Form 990-EZ and the
Form 990 or Form 990-EZ cannot be filed electronically (it has to be mailed), AND the state form must
filed electronically. (In this case, you need to file the state form independent of the
Form 990 or Form 990-EZ and include a pdf copy of the Form 990 or Form 990-EZ).
Below lists each state form and the restrications that apply.

New York CHAR500 Annual Filing for Charitable Organizations
- If filing a CHAR500 without a Form 990 or Form990-EZ, it must be for filing
year 2013 or later.
Hawaii Initial Registration
- Must NOT be filed with a Form 990 or Form 990-EZ.
- And must be filed electronically.
- And it can only be filed once.
Hawaii Annual Charity Transmittal Form
- Must be filed electronically.
- And the Hawaii Initial Registration must already have
been filed using efile.form990.org.
Michigan Initial Charitable Trust/ Charitable Solicitation Questionnaire
(includes the Registration Statement and Inventory forms)
- Is filed only once. DO NOT FILE this form if you are already registered with Michigan.
Michigan Registration To Solicit Donations
- You MUST BE registered with Michigan if you file this form.
